[vi], (74) pp. Title page, frontispiece printed in green ink with a ruled border, 9 wood engravings in green by Gaylord Schanilec, tailpieces of typographic ornaments, 6 end-notes by Douglass S. Dark green cloth spine, printed paper top cover and spine labels, slate-green end-papers; binding square and tight.
